Fork-lift tip-over kills operator - Worksafe Victoria
This tragedy highlights the need for forklift operators to wear a seat-belt.
A Coronial Inquest was held and in his findings the Coroner stated:
" …the use of seatbelts in these circumstances is critical to assist in reducing the risk of injury".
The Occupational Health and Safety (Plant) Regulations 1995 requires employers to undertake hazard identification, risk assessment and control risks so far as is practicable. The Code of Practice for Plant provides guidance on this.

Seat Belt Usage Saves Lives
The evidence is absolutely overwhelming that seat belt usage saves lives. This applies both on public roads and at work in any type of mobile equipment. The use of lap/shoulder belts reduces the risk of fatal injury by 45% and the risk of moderate-to-critical injury by 50%. For light truck occupants the numbers are even higher being 60% and 65% respectively. Jurisdictions that have and enforce seat belt laws significantly reduce the rate of fatal and severe motor vehicle injuries.

Construction fatal
While reconstructing a road, this worker turned his crawler tractor around and was returning to the storage area to refuel. Travelling on relatively stable ground he drove the left track off the road and the machine rolled down a bank into the creek. The machine struck large boulders in the creek bed and the operator was thrown from the cab. He landed on one of the tracks, was carried forward and crushed against the frame, receiving fatal injuries. He was not wearing a seat belt.

EQUIPMENT OPERATOR FATAL
While land clearing, the crawler loader operator drove too close to the edge of a 45 degree slope. When the soft ground began to give way, he tried to back up and the loader rolled onto its left side. The operator, who wasn't wearing a seat belt, tried to jump clear but was pinned between the ROPS canopy and the ground.
